Enrollment Procedures

To comply with state rules/regulations, the following forms must be received before leaving your child in our care.

1. Application (This form must be completed leaving NO blank spaces. Please place N/A if not applicable)
2. Discipline and behavior management policy signed and dated.
3. Infant feeding schedule (if applicable)
4. Infant safe sleep policy (if applicable)

State rules/regulations allow a maximum of 30 days for the following forms to be returned to the office.

Children’s Medical Report (Part A to be completed and signed by parent/guardian. Part B to be completed by a physical examiner. On reverse or attached the child’s immunization history.)

**Note: If not received within 30 days of enrollment, child cannot attend until this form is provided.

Additional forms Bright Beginnings Child Development Center requires to be returned prior to leaving child in our care are as follows.

1. Permission for Christian education (non-denominational)
2. CACFP income eligibility form
3. Permission to play outside of the fence
4. Acknowledgment form

Once you have decided to enroll your child at Bright Beginnings Child Development Center a $25.00 application fee is due in order to retain a space for your child and at that time you will receive a parent packet containing all the above listed forms including duplicates for your records of the Discipline and Behavior Management, the Infant safe sleep policy and a copy of the NC Summary of Child Care Laws and Rules

Bright Beginnings Child Development Center enrolls children from the ages of Birth – Twelve Years Old.

Tuition Fees

Infants	$150.00 per week
Toddlers (1year olds)	$145.00 per week
2 years old	$135.00 per week
Preschool (3-start K)	$130.00 per week
Before/After School & Head Start Care	$85.00 per week
Full Day Coverage for School Age, an additional	$20.00 per week
Full Day Coverage for Head Start, an additional	$20.00 per day
Summer Care (school age)	$120.00 per week
Head Start Summer Program will be according to the child’s age.

Late Fees

Late Payment	$10.00
Late Pickup/Early Drop Off	$1.00 per minute

Activity Fees

*Field Trips will have a fee. As soon as we know what the fees are for each field trip we will make you (parents/guardians) aware of the amount.

Sheet Fee (wobbler class thru explorer class) is $1.00 per day.

Diaper/Pull-up Fee is $1.00 each.
